Permission for Release of Records Archdiocesan form for grade school records to be sent to Queen of Peace High School. A parent/guardian should give this form to the child's elementary school principal, guidance counselor or teacher. This allows QPHS to receive a copy of the student's academic records and standardized test scores. Griffin for a Day Permission form to visit Queen of Peace as a "Griffin for a Day". ATHLETICS Medical Release Form The school must have on file a medical release form signed by the parent/guardian. A separate form is required for each sport each season. If a student plays 3 sports, the parent/guardian must submit 3 completed forms, one for each sport. Medical Examination Form In order to participate in any athletic program, every student must have a physical on file with the school. Physicals expire after one year. If a student has a physical which expires on October 2nd and he/she is on a fall sport, he/she will have to sit out starting October 3rd until a new physical is obtained. This form is mandated by the NJSIAA in order to participate in any interscholastic sport or activity. The form is to be signed and dated by both the student and guardian.
